4 以外は正しいです。
4 ですが、「正しい形」ということはもとになる語が与えていると思います。
laid にした、ということは lay とあったのでしょうか。
lie - lay - lain
lay - laid - laid

lie と与えられ、lying とするのが正しいと思います。
lay と与えられていたのであれば出題がおかしいと思います。
