機械操作とトラブル発生云々「trouble-shooting」の話ですよね、ちょっと技術屋さん英語ですが、かなり正確ですよ。

相手方により明確に伝わると判断されたら[improper/or irregular]を入れて置けば如何でしょうか?

preceding operation procedure 先行する操作手順
reason/s supposed一つか一つ以上の(思い当たる)原因
to have caused は to have induced　でもイイかと思います!
