報酬制禁煙プログラム訳(2)

先ほどの質問の続きです。
報酬制プログラムは有効だけれども問題なのは「ゲーム」になっていることである。
そのゲームとはそもそも喫煙者でもないのに喫煙者と偽っている人たちのお金目的で参加してくるというケースである。というような文章が書いてあるなかで、ある実験ではこういう結果が示されたという文に次の文章が続きます。

(1)An estimated 20% of those recruited were non-smokers, although this did not affect the study’s findings.
参加者のおよそ2割がそもそも禁煙車だったが、これは今回の研究の知見の結果には加味されていない。

(2)By contrast, a study of pregnant smokers who were offered participation in a financial-incentive scheme revealed that none of the 239 enrolled, all of whom were biochemically tested at baseline, were non-smokers.
対照的に、報酬プログラムに参加を求められた妊婦の喫煙者のある研究では、参加した人の中でそもそもたばこを吸わない人だったという人は239人の中で一人もいなかった。

(3)This raises questions about the contextual features of incentive schemes that can be used to minimize gaming.
報酬プログラムのこれらの実験の前後関係により、ゲームとして参加しているのではという考えを過小評価しているのではないかという疑問が生まれる。

そのあとの文章ではプログラムは恵まれない国や低所得者たちにはものすごい禁煙の持続率を保つ力があるといっています。

段落が変わり、

薬剤治療や禁煙促進の介在による報酬を伴わない計画では、参加者は恐らく禁煙をしたいと願う参加者ではある。しかし、我々はお金に興味があり、特に手の届かないお金を欲しがるものである。

と訳しました。ここの訳は難しかったです。

(5)Given their equal effectiveness to other interventions, financial incentives are less acceptable to the public, and in turn potentially to health professionals and policymakers, than other interventions for changing behaviours.
これらの結果はほかの治療介在と同じように効果があるが、他の禁煙促進プログラムに比べて報酬プログラムによる禁煙というものは好況に認めてもらうには難しく、また、専門家や政策立案者へ認めてもらう可能性も低い。

(6)Why is there not greater acceptability of a seemingly cost-effective intervention for reducing the leading cause of preventable premature death worldwide?
世界的な早死にを引き起こすことを予防するうえで経済的にも効果的に見える方法を認めないのはなぜだろう。

でいいのでしょうか。
why is there notの形が見慣れず困っています。一応訳してみました。

　後段の解釈は逆で、（禁煙には100％成功する）非喫煙者のプログラムへの参加はあるものの、Halpernの論文の主張に影響するほどではないという意味のようです。

　問題ありません。参加者239名全員が喫煙者だったということですね。

　問題ありません。おそらく、Helpern論文では調査の全体を通して、利益ゲーム（報酬目的）的な参加を排していると匂わせているのでしょう。本当にそうなのか、とこの筆者は疑問を持っています。

　倒置表現の仮定法過去ですね。

「このようなスキームが薬物や金絡みではない（認知）行動療法であったなら、禁煙を望む喫煙者への呼びかけへの介入試験の範囲内に収まっていたことは疑いなかっただろう（に）。」

　We are funny aboutが、moneyとmoney out of place両方にかかっています。

　おおむね問題ありません。Givenは仮定するという感じかもしれません。「効果が他の禁煙治療と同等だったとして（も）」みたい感じです。

「世界的な早死にの防止可能な主要原因を減じるための、一見して費用対効果の高い介入方法が、他の手段より受け入れやすいとはいえないのはなぜか？」

　この前段でless acceptableと言っているので、その理由を述べるために疑問文としているようです。この直後に理由を挙げて、金銭によるインセンティブを批判していますね。

　There is not greater ～ because ～.でbecause部分を問うためのwhyと考えればいいでしょう。
