(1) This is a little complicated because there isn't one main entrance examination in America. I hope my explanation below suffices. There are two main college entrance exams, the SAT and ACT, required by most (UCs, CSUs, privates) but not all colleges. Usually a college will accept either one. These standardized tests cover math and english (however there is also a short science section in the ACT). If a student desires to demonstrate additional knowledge to colleges, they can either take AP or SAT II exams (which both serve to test students on more specific topics). The main difference between the AP and SAT II exams is that sufficient AP scores can translate to credit in college while SAT II exams are taken to improve college applications. There are probably other exams I am not aware of (I'm pretty sure I took one towards the end of high school to prove sufficiency in math and english but don't remember what it was called). (2) I know the years of required math can vary by school, but I'm sure all high schools require math. It is also a prominent aspect of the exams I mentioned above.
